Completed application form (typewritten or legibly printed) with candidate's signature, sent directly to the appointed auditions representative on or before the established deadline. (Unsigned or incomplete applications will not be accepted.)

Certified copy of candidate's birth certificate or other legal proof of age such as driver's license or passport. Applicants for the current season of auditions must be 20-30 years old for all voice types.  Applicant's Birthday must fall on or between August 1, 1981 and July 31, 1991.

Non-refundable application fee of $30.00 in the form of a check made out to:  "MONC/<your district>"

A Resume and a 8x10 black and white photo is required.
